Understanding the Landscape

What Does "Canadian-Friendly" Actually Mean?

Most US medical schools classify applicants into three categories: in-state, out-of-state, and international. For Canadians, the distinction that matters is whether you are evaluated as an out-of-state applicant — competitive, lower tuition tier — or as a true international applicant — higher tuition, separate pool, much lower acceptance rates.

A school is Canadian-friendly if it formally treats Canadian applicants the same as out-of-state US applicants, or regularly admits Canadians and accepts Canadian coursework toward its prerequisites. This data comes directly from the AAMC's Medical School Admission Requirements (MSAR), updated every cycle.

Policies change every cycle.

Always verify each school's current Canadian applicant policy directly on their admissions website before submitting. The MSAR is the authoritative source, updated annually each spring.

The core advantage for Canadians:

Canadian undergraduate coursework is accepted at virtually all US medical schools without additional verification — except French-language Quebec transcripts, which some schools may require translation for. You do not need to repeat courses taken in Canada.

Academic Preparation

Standard Prerequisite Courses for US Medical Schools

While each school sets its own requirements, the following core courses are required or strongly recommended by the vast majority of US medical schools. Canadian coursework at accredited universities satisfies these requirements.

CourseTypical RequirementNotes for Canadians
Biology (General)2 semesters + labCanadian 1-year bio with lab accepted universally
General Chemistry2 semesters + labYear 1 chemistry at Canadian universities accepted
Organic Chemistry2 semesters + labSome schools accept 1 semester + biochem in lieu of 2nd semester
Biochemistry1 semester (increasingly required)Check each school — many now require this separately from orgo
Physics2 semesters + labLife sciences physics accepted; calculus-based also accepted
Statistics / Math1 semester (most schools)Statistics more commonly required than calculus now
English / Writing1–2 semesters (private schools)Duke, Georgetown, Dartmouth — verify per school
Psychology / SociologyRecommended for MCAT alignmentNot required as a course, but MCAT Psych/Soc section is tested
AP Credits and Online Courses:

Policies vary significantly. Many schools accept AP credits for non-science prerequisites but require in-person lab courses at a university. Some schools (e.g. Northwestern, UCSF) accept online science courses; others do not. Check each school's MSAR entry for their specific policy.

Critical Planning Information

What Canadians Must Know About Residency

This is the most consequential factor most Canadian applicants underweight when building their US school list. A US medical degree does not provide equal access to Canadian residency programs.

The residency match reality:

In 2025, 94.7% of successful Canadian residency matches (first iteration) were graduates of Canadian medical schools. Some Canadian residency programs do not consider US medical school graduates at all. The Association of Faculties of Medicine of Canada actively advocates for Canadian graduates in the match.

If your goal is to practice in Canada long-term, a US medical degree creates meaningful barriers to re-entry. If you plan to practice in the US — or are genuinely open to it — a US degree is the direct path.

Dr. Gill's recommendation:

Have a clear answer to "where do I want to practice?" before committing to a US application strategy. If the answer is Canada, Canadian schools should remain your primary strategy. US schools are a considered parallel track — not the default backup.

Common Questions

Frequently Asked Questions

Do Canadian GPAs convert the same way?

AMCAS converts Canadian grades into a standardized GPA scale through its verified transcript process. Competitive US applicants average around 3.7 GPA — the same standard applies to Canadian applicants.

Do I need any US coursework?

Most US schools accept fully Canadian degrees and coursework. A small number require or prefer some US educational experience — check each school's MSAR entry individually. There is no blanket requirement.

Can I use my MCAT score taken in Canada?

Yes. MCAT scores are accepted uniformly regardless of where the exam was taken. There is no distinction between a Canadian-administered and US-administered score.

What about financial aid?

Canadians are not eligible for US federal financial aid. Most private scholarships are also restricted to US citizens. Plan on Canadian bank lines of credit and provincial loan programs. Budget approximately CAD $90,000–$98,000 per year in tuition alone.

What is the MSAR and should I buy it?

The AAMC's Medical School Admission Requirements (MSAR) is the definitive annual database of US medical school requirements, acceptance data, and applicant policies. It costs approximately $28 USD and is the most important tool for verifying current Canadian applicant policies.
